Please arrive 15 minutes prior to your appointment time to ensure a smooth check-in. This keeps our office on schedule so that we can see everyone at their scheduled appointment time. We understand that everyone leads busy lives and being late can delay not only your examination, but to everyone else scheduled later in the day. We reserve the right to cancel your appointment if you are late.
Insurance Information:
If you plan on utilizing insurance, make sure our office has received and verified your insurance information before your arrival. We will reach out for more details in order to verify your coverage via text or phone call. Failure to respond prior to your appointment will result in cancellation.
